How does the bitcoin transaction fee get paid to miner(s)?

The current "cheap" rate for a bitcoin transaction is $22.

How does this get paid to the miners? When I view a blockchain explorer a few days later, there might be 16 confirmations.

How many confirmations does it take to get added into a block?

Is the $22 split amongst the 16 miners that confirmed the transaction?

I understand proof of work. But I don't quite understand how a transaction fee gets paid out. Thanks.
